PURCHASE PAYMENT PLAN AGREEMENT

Agreement: I, ____________________________________________________, the buyer,

understand that I am purchasing a(n) _____________________________________ parrot.

The purchase price is ______________. The shipping costs will be $_________________.

Terms: I agree to pay a down payment of $____________ (Enclosed with this agreement)

And _______ payments of $____________ every ____________________ (week, 2 weeks,

month) until the purchase price of the parrot (plus the shipping cost) is paid in full. I further

understand that, if any payments are not received within 10 days of the terms agreed to

above, this agreement is cancelled.

Cancellation: I understand that I have five days after the date of the down payment to

notify J-Birds in writing of my intent to cancel our agreement. If J-Birds receives this

letter of intent to cancel within five days of the date on the check or the date of the

credit card transaction they will refund the amount of my deposit less a $25 handling fee.

If I cancel the purchase after this five day grace period I understand that all funds paid

will be forfeited.
